On the first day of the great international kickabout in South Africa, and as a lorry-load of national anthems and novelty records flood the airwaves and the internet, everywhere from South Korea to Spain, it's worth remembering the musical inspiration that the beautiful game has had on composers whose ambitions stretch even further than Chris Waddle's on Top of the Pops. So here's my pick of the football-indebted pieces you won't be hearing on the terraces this summer …

The first-ever football chant written by a great composer was by Elgar in 1898, whose devotion to Wolverhampton Wanderers is commemorated with a plaque at the club's Molineux ground. Elgar saw the phrase “he banged the leather for goal” in a newspaper report of a match, and was so inspired by this quaint locution that he set it to music – this music, to be precise. Not sure it ever actually caught on at Molineux, but Wolves's fans reportedly sing Nimrod from Elgar's Enigma Variations at home matches.

The same year (1898 was a corker for sports-related music), Charles Ives composed the single best two-minute condensation of a football match (OK, American football) ever written. This is his Yale-Princeton Football Game, marking the Yale Bulldogs' 6-0 victory over the Princeton Tigers. Everything's in there, from Yale chants, to dodged tackles, and the concluding touchdown. A terrific performance this too, from the Malmö Symphony Orchestra and James Sinclair on YouTube.

Back with euro-soccer: Bohuslav Martinu's Half-Time, a Rondo for Large Orchestra, was composed in 1925 as a football-crazy round-dance, and the earthiness of Poulenc's Gloria came to him, the composer said, after seeing “some serious Benedictine monks … revelling in a game of football”.

Among today's British contemporary composers, there's a rich seam of football-related works: Arsenal fan Mark-Anthony Turnage includes a terrace chant in his orchestral piece Momentum, and there are football scenes and references in his operas Greek and The Silver Tassie. James MacMillan is a celebrated Celtic fan, who is unafraid to involve himself in some Rangers-Celtic chant-related stooshie, and there are traces of the terraces in the football rattles in his Britannia, an orchestral work from 1994. Michael Nyman has done more than both of them, though: there's his Beckham Crosses, Nyman Scores, which uses John Motson's commentary as the basis of a Different Trains-style fusion of sampled speech and instrumental music, and After Extra Time, a whole album of football-related pieces that takes in a musical five-a-side between “Riff Athletic and Riff Rangers”, a homage to Nyman's beloved Queen's Park Rangers, and a memorial to the Juventus fans killed at the Heysel stadium in 1985.

There are many other musical connections with football, like the games nostalgically remembered by AE Housman in his poem, Is My Team Ploughing, and set in my favourite musicalisation of it by George Butterworth; the fact that critic, musicologist, and radiophonic guru Hans Keller was a passionate football fan and wrote a book called Music, Closed Societies and Football; and that Percy M Young was a biographer of Handel, Vaughan Williams, Elgar – and Bolton Wanderers. Thanks to Radio 3 producer, and Aldershot Town fan, Lindsay Kemp, for these last football-related thoughts. You will have your own, so let's hear them. And if you want to hear a genuinely well-crafted bit of football-pop, with nary a perm in sight, my mate Jono Buchanan has written a new song that was part of the World Cup Kick-Off concert in South Africa last night – you can hear Believe here.

I have been watching the country music awards and have many of the first ones taped…actually some in Beta that I can’t watch anymore. I try to never miss any of them as the entertainment is so great and of course I am such a fan of country music. Unlike most of the other music award shows…the country award shows are always GREAT!
Until this 2010 CMT Award show last week. What a major disappointment.
I was ashamed that the show was on to be honest… Kid Rock brought a side of country that none of us can be proud of. If CMT is trying to appeal to the other genres to get more people to watch…maybe it worked…however I have to believe they lost a lot of the country people.
First of all..don’t understand why he was host. BUT it would be okay if he was good….or looked good….or acted good. He did NONE of the above. The crummy dirty looking baggy jeans and stupid hat… smoking…. drinking… pretty much acting like he was drunk, did NOT do country music a favor. The ad-libs were TERRIBLE. The camera kept going to the audience…didn’t see a lot of people there laughing either….some looking stunned! Maybe those country artists care more about the country music reputation.
The blond whoever she was doing interviews was almost as bad. Was she drunk or what? Had she even an idea of what she was going to say? I don’t think so.
All in all…. it was the very worst country awards show I have ever seen…..I hope CMT rethinks what is important.
Oh…how about Toby Keith that STARTED to be nice in his interview and say that the opening act was good…then thought twice and even though he gave his first answer, decided to be COOL by saying ” they were in the bar and didn’t see it!” Sorry Toby….wrong answer.

It's an experiment creating a new (for many, if not for people of the Dolby/Beatnik era, or people on lists like microsound) way to bringing people together for mutual projects … which can only be beneficial for many voices which would otherwise go under-heard, under-recognized. (I can recall one such talented voice popping up a decade ago in Turkey … a guy who went on to win awards and a pro career because his gifts were heard online by people in a position to recognize them.)

If the results were weak (I thought the 'Tripwired' piece was quite good considering what they had to work with), that's partly owed to the choice of materials people had to work with … and to the low # of entrants. It's a start.

In the past such projects were done by mailing lists and with 1 or 2-month timelines. The day is approaching (see here e.g.) when stuff like this will commonly be done live, online, with day or two timelines … and an audience – probably some very large audiences.

If stereotypes held true, you would think that the Republicans would be the ones telling folks to turn that blasted music down. But this year — and indeed in many past election cycles — it's the GOP that has been attracting cease-and-desist letters for pilfering music against the artists' wishes. So let's take a look at some of the more notable GOP music fails from this cycle, and cycles past.

Senate candidate Chuck DeVore (R-CA) got burned for using for using altered-lyric version of Don Henley's “The Boys of Summer” and “All She Wants To Do Is Dance” for his campaign's Web ads. David Byrne is suing Gov. Charlie Crist (I-FL) for using “Road To Nowhere” in a Web ad during his previous Republican Senate primary fight, and of course, as we reported yesterday, Rush cut to the chase and told Senate nominee Rand Paul (R-KY) to stop playing “Tom Sawyer” and “The Spirit of Radio.”

The Orleans song “Still The One” has a special place in politics as it has been used not once without the band's permission, but twice — and the first instance contributed to its author's entrance into national politics. In 2004, the Bush campaign used the song at a rally. As the song's main author, John Hall, told MSNBC in 2008: “George Bush was busy campaigning on an 'ownership society,' yet never asked me, the band, or the publishers for permission.” Hall and other stakeholders in the song quickly sent a cease-and-desist letter, and the Bush campaign dropped the song.

In 2006, Hall went on to be elected to Congress as a Democrat, defeating an incumbent Republican — an event that was spurred in part by his experience from 2004. “It was one of the things that got him even madder,” Hall press secretary Tom Staudter told TPMDC. And then in 2008, the Republicans used the song yet again, this time the ill-fated John McCain campaign. “This is yet another example of John McCain not learning anything from George Bush's mistakes,” Hall told MSNBC, also adding: “The only one John McCain is Still the One for is George Bush.”

But that was only the tip of the iceberg for the McCain campaign — which was practically a walking Limewire setup. Jackson Browne also sued it for using his song “Running On Empty” in an ad, for which the two sides later reached an out-of-court settlement. Van Halen objected to McCain's use of their song “Right Now” at a rally. The Wilson sisters from Heart strenuously objected to his campaign's use of “Barracuda” to promote Sarah Palin. And finally, the McCain campaign used “Pink Houses” and “Our Country” by John Mellencamp, who sent a letter demanding that they stop.

Mellencamp had already been ripped off by the Bush campaign back in 2000, when they used “R.O.C.K. In The USA.” He asked them to stop — but that wasn't the end of it. “They said OK, and then used it anyway,” Mellencamp told the Indianapolis Star, pointing out that the Bush campaign only stopped after the national media made fun of them for it. “I think they kind of said, 'Oh, this isn't playing the way we thought it would.' So I think they quit using it — I think.” (The Indianapolis Star, September 3, 2000, via Nexis.)

The Bush campaign also used the Tom Petty song “I Won't Back Down,” and the Sting tune “Brand New Day” — and was told in each case to stop. Randall Wixen, the publisher of “I Won't Back Down,” told the Bush camp to stop after being asked to do so by Petty's management, saying that the use of the song “creates, either intentionally or unintentionally, the impression that … [the Bush] campaign have been endorsed by Tom Petty, which is not true.”

Four years earlier, the Bob Dole campaign was threatened with a lawsuit for its rewrite of the 1960s soul classic, “Soul Man,” turning it into “Dole Man,” much to the chagrin of the song's publishers at Rondor Music International. The campaign agreed to stop using the jingle, as the Chicago Tribune reported in September 1996: “Rondor announced on Tuesday that it had warned the Dole/Kemp campaign it could be held liable for damages of $100,000 for each unauthorized use of the song, plus legal costs, if a satisfactory settlement was not reached.”

Perhaps the most famous music fail was President Ronald Reagan's invocation during his 1984 re-election campaign of Bruce Springsteen — Reagan did not actually play Springsteen's music, but tied his brand to the Boss's in a speech. “America's future rests in a thousand dreams inside your hearts,” Reagan said. “It rests in the message of hope in songs of a man so many young American's admire: New Jersey's own Bruce Springsteen. And helping you make those dreams come true is what this job of mine is all about.”

Springsteen fired back by declaring at a concert that he doubted Reagan ever listened to the “Nebraska” album. Then the Democrats tried to claim Springsteen's support, only to have the Boss make it clear he was not endorsing anybody. (He later hit the campaign trail for John Kerry in 2004, and Barack Obama in 2008.)

Another music fail by the Reagan campaign involved their unsuccessful attempt to play John Mellencamp's “Pink Houses,” with its seemingly patriotic refrain, “ain't that America” — and which is actually a cry of protest against a world of haves and have-nots. The Reagan campaign never ended up playing it, though — unlike the Bush and McCain campaigns, they dutifully sought Mellencamp's permission, which the left-wing rocker refused. Mellencamp commented on the event in 1986, telling Newsweek: “Ronald Reagan appeals to people's emotions, and not to logic. That's what a guy in a rock-and-roll band does. Besides, if a dumb kid from Indiana can come from nowhere and have a hit record, it can happen to anybody. Isn't that what America is all about? That's what Reagan wants to represent. But that's not logical. That's not real.”

Nearly one-in-four mobile users is now toting around a smartphone, according to the latest finding from Nielsen.  But activities like streaming audio, ringtone downloads, and full-track downloads lag far behind a range of other smartphone activities.

Perhaps unsurprisingly, more functional activities like email, mapping, text messaging, and mobile internet surfing are most demanded among smartphone consumers.  But other less-obvious categories like picture downloading, IMing, and gaming beat out music activities, though high-ranking 'app downloads' certainly encompass music-specialized services (a complete breakdown of all ranked activities here).

And the war among smartphone makers?  On a quarter-to-quarter basis, Nielsen's Q1 stats showed a slight, 2 percent gain for the iPhone (to 28 percent), and a concomitant 2 percent drop for Blackberry to 35 percent.  Android gained 2 percent to reach 9 percent overall penetration, while Windows Mobile slipped 2 percent to 19 percent overall (a complete breakdown of smartphone rankings here).

Kid Rock will perform during the Nightly Concerts at LP Field on Friday, June 11.

On Wednesday, “The Sixth Annual CMA Music Festival Kick-Off Parade” starts at 11:30 AM. Immediately following the parade, “The Fifth Annual CMA Block Party” will take place on the Chevy Music Stage on the Bridgestone Arena Plaza (at the corner of Fifth Avenue and Broadway).

CMA Music Festival and the Nashville Convention & Visitors Bureau have partnered to present the Music City Zone. Located at the Public Square Park at the top of Second Avenue, the Music City Zone will have free daytime performances showcasing an eclectic array of musical styles on the GAC Stage. Open Friday, June 11 from Noon-5:30 PM/CT; Saturday, June 12 from 10:00 AM-6:00 PM; and Sunday, June 13 from Noon-5:30 PM.

Society of Leaders in Development (SOLID) will donate a portion its Yard Sale proceeds to the MusiCares Nashville Flood Relief Fund. SOLID’s 8th Annual Yard Sale will take place Saturday, June 12 at the Bridgestone Arena.

The Grand Ole Opry has plenty to offer fans this week, including tonight’s Tuesday night Opry at 7 p.m. with a performance at Municipal Auditorium in downtown Nashville. Among those scheduled are The Band Perry, Jimmy Dickens, Josh Turner, Carrie Underwood, and Trisha Yearwood.

The Country Music Hall of Fame And Museum has tons of autograph signings and more to bring in festival-goers.

Music lovers who illegally download mp3 music from the internet are the bane of record producer’s lives. The process has thrown the music industry into admission of huge drops in profit and is an on-going battle.
New techniques are currently being trialled to combat the problem of the illegal mp3 music downloading. One is being backed by massive R & B producer, Timbaland. Recognising that people want cheap, affordable ways to get their mp3 music and using the latest technology to provide this, he has joined forces with Verizon Wireless network in a ground breaking deal to release mobile only music.
Timabaland will release one track per month, only available to subscribers of V Cast, Verizon’s mobile entertainment service. Consumers who want to obtain mp3 music often do so illegally, costing the music industry millions but it has been found that they are more willing to pay anything up to 3 pounds for a track downloaded to their mobile phone.
R & B songs are the hot favourite for music downloads and ringtones of late and Timbaland will be putting his tracks into the public domain for a bargain price of 1 pounds per track. It is perfectly legal to then transfer this music to a computer, thus reducing the need for the more risky way to collect mp3 music.
It seems music downloads to mobiles is the way to go as these types of sales rose by 32% in Japan last year, bringing with it an 87 pounds million industry. The UK has the largest concentration of mobile phones which are music enabled but consumers are restricted by download prohibitions. Leading the way here is Nokia who are to provide a service with unlimited downloads and it is hoped this will kick-start the industry here.
More legal solutions to consumers who wish to download mp3 music are coming from the Far East with Google’s new feature it is preparing to add to its Chinese search site. It is a system whereby it will be legal to download music and keep it in whichever format you wish. The money will be made from advertising. The idea is to use free music to help sell ads and then split the proceeds with music copyright owners.
It is reported that Google have already managed to get Universal Music Group on board with this idea but some believe this to be questionable. They are currently trying to gain the support of other music companies and calm their fears that they will lose out. Legitimate music sales in China are minimal so it really shouldn’t affect the business that much, other than to improve it.
More news from China regarding the consumers who like to download music is the latest invention dubbed an ‘ePod’ or ‘green Pod’. It comprises a wind up mp3 music player that is environmentally friendly. Saving on electricity usage this latest gadget can be recharged by the use of a wind up handle on the back.
One minute’s worth of winding can be enough to support 40 minutes of music listening and can play for an incredible 20 hours before needing a full charge again. The Eco Media Player can store up to 500 songs and also doubles as an emergency mobile phone charger, useful until they bring in the wind up mobile phone that is currently being worked on.
It would seem that the new advances in the industry should benefit fans who are looking for mp3 music as well as record companies and the environment. Consumers should be able to download music as cheaply as possible while remaining within the law, record companies have a choice of ways to supply music while still making a profit and with a wind up music player or phone we can all do our bit for the environment.

How to download music from an iPod Nano

Apple Computer is doing its best to download music for your iPod Nano as simple as possible, but also frustrating and annoying because of file permissions in contradiction with the activity. My hope is that this short guide can help you.

The easiest way for your iPod with iTunes for music. The Apple program is free and works well on both PC and Apple (of course). This program can convert songs to MP3 files protected apple ACCM4P format will not transfer from ACC to MP3 automatically, in other words, you can download MP3 files in iTunes, but you can not convert the songs in iTunes on a PC without a little 'further. (This is a file converter like Blaze Media Converter AVS Converter or both must be free). If you follow this guide to save time and stress.

Download music on iPod:

Start! (This step is optional, but recommended because it allows forIPod is a PC and Mac Open iTunes and go to "Advanced" at the top, that the import-go:

Select MP3 as the standard format such as AAC and you'll be in good shape. Another note here: the songs from sites like Napster and Rhapsody, but not the iTunes download. The iTunes Store uses the ACC format and would need a converter to use the above. Not a big deal, but it's a step.

DownloadingTitle:

Step 1 Open iTunes (if not now the time) to download. Put your nano at this time.

Step 2 Rip from a CD: Insert the CD into your computer and after a few seconds in iTunes. All you have to do is click on the desired title and click Import CD "icon at the top right. Loads of songs in the iTunes library. It may take several minutes depending on the speed of your computer.

Step 3 ConnectYour nano. iTunes will be seen in a few seconds to load your songs automatically.

Note: The file will automatically save music in your iTunes library. If you have a new playlist that you can do by going to File "and" New Playlist "You can insert the name of the playlist. After naming your playlist, you will see in the left column. At this point You can select songs from the library, drag them into your playlist left again.

Caution: When using aFree download site or a peer 2 peer (P2P) site is free, do not always know what you get. In other words, the songs may contain spyware, trojans or viruses or worse. So I suggest you subscribe to a download site, which is cheap, but safe.

Lunchtime Lowdown: Miley Cyrus Wrote Music About Ex Nick Jonas; Tara Reid Shows off an Improved Bikini Body

The 38th International Istanbul Music Festival, scheduled for June 3 to 30, is putting a youthful face on the classics this year. Innovative musicians like Lang Lang, the colorful and kinetic 27-year-old Chinese pianist, are among the draws at this year’s festival. He’ll perform works by Frederic Chopin and  Robert Schumann, in keeping with the festival’s grand nod to tradition by celebrating the 200th anniversaries of the composers’ births.

Adding a twist to what’s fresh among the classics, the 75-year-old composer Arvo Part is joining the youth parade by offering a world premiere of his work, “Adam’s Lament,” performed by the Estonian Philharmonic Chamber Choir and the Borusan Istanbul Philharmonic Orchestra, conducted by Tonu Kaljuste.

Other firsts in this year’s lineup: Riccardo Muti, the Italian conductor, will lead the Vienna Philharmonic; and the Romanian pianist Radu Lupu will perform in Turkey for the first time.

well, i(''s sunday afternoon and i've got time enough to respond, which also allows me to avoid working on a new draft, although there isn't much meat to chew on here. if one was promised a wireless syncing app when they purchased a phone and it was not available, seems like grounds for a complaint to the seller for misleading advertising and if you're steamed enough, then organize and rally and get a class action lawsuit going. but the doj? hmmm?

my pops is a sports car nut and has had severa. he wanted the morgan company to add some features that some other cars had, but they wouldn't. of course, he could go to an indie and get it done, then making his warranty void. it's not really my business what the standards of approval are, anymore than morgan has to spell out why they won't install his carb change. they like the morgan the way it is produced and want to keep it that way. it is there biz.

if my music store didn't carry every thing i personally wanted, then i probably have desires beyond what my store offers and it sounds like you have a lot of other gripes as well. there's no limit to what people want. there were tons and tons of gripes about free apps not being perfectly executed, about having ads on them, so people will always complain, it seems. maybe you have some other points which might possibly hold legal merit, but this example is a far cry from one.

Deaf people may not be able to hear music, but soon they might be able to feel it, even on the go. A concept device from German designer Frederik Podzuweit promises to make hearing music a reality for the deaf community based on synesthesia — a type of sensory stimulus evokes the perception of an entirely different sense.

Music for Deaf People hopes to turn music into the same emotions and feelings the non-deaf hear by using electricity to stimulate a special membrane on the device. Music for Deaf People can project the vibratory feelings of bass, midtones, and treble from any portable audio device to a device that sits on a persons neck as headphones would when resting on shoulders. The volume or track number adjusts by use of the large, easily accessible controls right on top. With a clean futuristic design, even audiophiles looking for a new way to listen to music might find it interesting to try.

Now let me be clear from the very start so there is no confusion… I’m on David’s side on this one, as I imagine many of you reading will be too. Not to say there isn’t any bias but I pride myself on being as objective as possible and I believe I’ve logically come to my conclusion.

You see, companies such as WMG has beef with Youtube. They have a history of conflict, just Google the words “WMG” and “Youtube” and you get a bunch of results on the issue. In an over-simplified summary, WMG doesn’t believe they are getting their fair shair in the online-streaming business and as a result, they heavily police the internet, taking down all copyright infringement material. OK, that in itself is their prerogative… let’s be honest, they deserve to be paid for the music they own/protect on “behalf” of their artists. As much as I love social media, and the mass availability to new music, whether legally or illegally, I understand that there are copyright laws for a reason and that these are businesses trying to stay afloat.

But to attack an artist for posting cover songs that aren’t even for sale… I think that’s crossing the line. Their argument is that David is sharing music that he doesn’t own and allowing the entire world to hear while WMG doesn’t see a dime. If this were an issue of file-sharing the original songs and posting them all over the web then sure, that definitely has ramifications on sales and their bottom line. But the labels have decided to ignore the fact that David artfully recreates these covers with his own arrangement, instrumentation, and vocals and merely keeps the heart of the melody and the lyrics in tact nor does he charge a penny for them. So what exactly are they losing in this case?

Are they afraid that his covers are actually deterring people from purchasing the original songs? I wonder if any market studies have been conducted to tangibly show a direct correlation between the popularity of a David Choi cover and the sales figures of a single. I imagine they are presenting the argument that it’s an inverse correlation, with rising DC views equating to falling sales numbers… but if anything I’d argue the opposite. If American Idol has taught us anything, it’s that when the public sees an artist they like covering a song, sales of that song spike in the immediate short term. I imagine the effect, though no where near as dramatic, is similar with David.

So again, what are they losing? DC potentially is helping their sales, plus they have the option of running ads against all the videos in question… all bringing in revenue that they would not otherwise have from an individual not even on their payroll. And I highly doubt this is a stance of principle. These companies exist to make money, not uphold artistic integrity.

And while yes, it’s clear that music sales in general have fallen over the years due to piracy and a crumbling business model, I don’t buy for one second that free online cover songs are the culprit. Bottom line is… well their bottom line. They don’t know how to recoup it so the want more money from Youtube. All or nothing. It’s almost like if McDonald’s sales on Big Macs were slipping so they started suing people at home for making their own two all beef patties, special sauce, lettuce, cheese, pickles, onions on a sesame seed bun.

WMG and all the other publishing companies, get over yourself. You’ve helped bring us a ton of music over the years, and for that we thank you… but don’t get into the business of taking away music from us too.

The next generation: Drew McIntyre, Sheamus, Morgan, RVD, Randy Orton, Anderson, even “The Pope” D'Angelo Dinero, and more, will never reach anywhere near Kurt Angle's level…nobody will! Kurt Angle, you're awesome, and when you do retire, you will be truly missed. Kurt Angle for life!!!

Beyonce Me Myself & I New Music Video – There is something rotten in the state of Denmark, but I just can not put my finger on it. Beyonce who is supposedly on vacation has premiered an alternative version music video for her song called “Me Myself & I” which was produced by Scott Storch and was featured on 2003 “Dangerously in Love” CD.

There is nothing spectacular about the clip other than the fact that in this version instead of going in reverse, the action moves forward and Beyonce changes her clothes couple of times.

Back to the fishy things going in Denmark, why is whenever Kelly Rowland has a new leak that surfaces, Beyonce releases something just minutes later?
Pure coincidence? Or Is she plotting to kill Kelly’s already dying career?

Part of a resident’s job is to learn the ropes in preparing for independent practice. While you’re a resident, you get the benefit of having someone looking over your shoulder to critique you as you determine how you are going to manage patients.

I frequently tell residents that different attending physicians practice medicine in different ways. Some practice defensive medicine more than others, some prescribe antibiotics more than others and some work harder than others. The resident’s job is to figure out whose practice they are going to emulate when they begin practicing on their own.

That being said, I usually practice conservatively. I don’t tend to shotgun a lot of cases. When residents present cases to me, I make them give me a differential diagnosis and justify why they order the tests that they order. If they can’t justify why they’re doing the tests, then I won’t approve the tests.

A resident rotating on the first day in our emergency department presented a case to me and his comments made me think.

A woman in her 40’s came in complaining of tender lymph nodes to her neck for the previous 36 hours. That was it. She had pain in her neck when she turned her head to one side and thought she had cancer.

The resident ordered a CBC, comprehensive metabolic panel, cardiac enzymes, coags, chest x-ray, urinalysis, influenza swab, and strep test. He wanted to know whether I wanted to do a soft tissue x-ray of the neck or a CT scan of the neck.

“So what do you think is causing the swollen glands?”
“Maybe strep, maybe cancer.”
“Why the cardiac enzymes and coags?”
“If it is cancer and she needs surgery, the surgeons require a baseline.”
“Any other symptoms besides the swollen glands?”
“Nope.”
“Why the urinalysis?”
“I figured they could do that while they’re getting the pregnancy test.”
“Why the pregnancy test?”
“She’s going to need x-rays, right?”
“We can’t do an abdominal shield?”
“Sure.”
“Is a $200 flu swab going to be worthwhile?”
“It could cause the swollen glands.”
“In a patient with no fever, no cough, no pharyngitis, and the incidence of influenza sporadic according to the CDC?”
“Didn’t think of that.” He was obviously getting annoyed. “Fine. What do you want me to order?”
“Anything else on the physical exam?”
“Not really. No nodes anywhere else. No signs of infection.”
“Let’s go look.”

I’m typing this case up on the fly and was going to finish describing the interaction, but then I thought that maybe you all would like to take a crack at guessing what was causing the bilateral tender lymphadenopathy in the patient’s neck.

Here’s what it was.

The point of the post was not to belittle the resident, but was more to make a statement about how another resident felt that residency training was lacking.

Another resident in our program lamented that most of their didactic teaching doesn’t involve close physical examination or a thorough history any more. She felt that the overwhelming teaching points during the residency program were to perform procedures and to work up patient complaints to avoid being sued: Take the patient’s chief complaint, order tests that can rule out all the things that doctors commonly get sued over, and have them follow up with their family physician.

You make the diagnosis – great. If not – that’s why they have family practitioners. Patients with high risk complaints and any risk factors for bad outcomes get admitted.

I actually got pegged as someone the residents like working with because I make them think about what they’re doing  – although the resident above avoided me the rest of the day.

If defensive medical practice is as entrenched in our residency programs as this resident seems to believe, our system will get worse, not better with health reform. More “insured” patients will be dumped into the system, health care access will become more disjointed, and patient will end up bouncing from emergency department to emergency department getting shotgun testing that will rule out remote life threats and protect the physicians from lawsuits but that will never really get to the bottom of the patient’s problems.

This patient probably would have had a high WBC count if labs were ordered. Maybe she would have been discharged on antibiotics and improved without making the diagnosis. The cost to the system for the proposed workup, though, would have been immense. Is this the way we want to spend our health care dollars?

Until we address the fear of malpractice that drives defensive medicine (and I’ll even cede that some of that fear is irrational), we’ll never reduce our healthcare spending.

Signing a Formal Agreement

In 2000, The Union and the Government of Malawi signed a formal agreement outlining the government's commitment to the CHLP .

Situation Analysis

A standardized evaluation of the epidemiology of childhood lung disease, the nature of the health services available to deal with them, and the efficiency, equity, and accessibility of these services was carried out .

Developing a Plan of Action

A 5-y plan designed to achieve countrywide CLHP coverage was prepared on the basis of available documents and the situation analysis. The plan included a manual of policies and procedures, a calendar of activities, a budget, a description of the responsibilities of the parties involved in implementation, an evaluation mechanism, and the procedures to be followed.

Establishing Initial Implementation Sites

Initial implementation sites (administrative districts) were selected according to the following criteria: presence of a functioning ARI and/or IMCI programme; commitment of the District Health Officer to the CLHP; availability of a designated health worker who, in addition to normal activities, assumed responsibility for implementation; and a catchment area of approximately 100,000 population. A plan for regular monitoring was then developed.

Training Health Management Personnel

The programme manager and staff at each implementation site participated in intensive training to enhance their management skills and the computer skills needed to efficiently manage information.

Training Inpatient and Outpatient Health Care Workers in Case Management

A CLHP training manual and modules were adapted to local conditions and used as a standard operating and reference manual. The training curriculum for health care workers, although focused on SCM of childhood lung diseases (pneumonia, tuberculosis, asthma, and HIV-related lung disease), also included case management of other major childhood illnesses (malnutrition, diarrhoea, malaria, anaemia, and meningitis) because children frequently presented with comorbid conditions. Annual training courses focused on theoretical and practical case management using local paediatric facilities and initially relied on an international course faculty. Local faculty gradually took on more responsibility for these courses. One-day follow-up training sessions took place 4–6 wk after the annual course plus ongoing in-service training.

Monitoring Progress

Information routinely collected to provide patient care formed the core of the CLHP monitoring activities. Information was gathered using standard forms . These documents were simple, clear, and kept to the absolute minimum required for adequate patient care and for monitoring and evaluating the CLHP. Monthly reports were generated on the number of pneumonia admissions by age and severity, treatment outcomes by age, and severity and monthly supply requirements.

This information was used for managing materials, for monitoring the quality of care, and for epidemiological surveillance. It was also used to assess health service utilization, the efficiency of the services provided, and the transparency of management procedures.

Focusing Services on Vulnerable Children

The target group of the CLHP was vulnerable children with severe/very severe pneumonia who were at greatest risk of dying and who required hospital care.

Managing Supplies and Materials

Uninterrupted supply of appropriate antibiotics was assured by using the routine reports to complete order forms to obtain annual supplies. To avoid “rupture of supply,” there was a “reserve” stock of medications (equivalent to 100% of the previous year's order) within the country. The information system was crucial for accurate planning of drug requirements.

Evaluating the Services

The monthly reports on diagnosis, treatment, and outcomes provided the information needed to monitor the CLHP. They were also used to evaluate output and to advocate for resources. Systematic review identified and corrected problems in implementation. Completeness of the records was routinely evaluated.

Technical support by external experts twice a year focused on maintaining quality. Unit managers also met regularly to review their work and their problems. In addition, an independent review undertaken during the third year of the implementation period provided in-depth evaluation of the services and of the role of all the collaborators in the agreement.

Scaling Up

Expansion was designed to achieve the widest possible access to the CLHP in the shortest time whilst maintaining quality. Five district/central hospitals were enrolled in October 2000 with five, six, and eight more enrolling in each subsequent year. By the end of the fourth year, all 22 district hospitals and two of the four central hospitals in Malawi had implemented the CLHP. Another central hospital was enrolled in the programme in 2004.

Results Achieved

From October 2000 to December 2005, 312 health workers (approximately one-quarter of all nurses, clinical officers, and medical assistants working in government hospitals) participated in 5-d training courses.

  • The 22 district and three central hospitals reported 48,702 cases between 1 October 2000 and 31 December 2005. The annual number of pneumonia admissions increased from 3,673 in the first full year (2001) to 7,516, 13,149, 10,134, and 13,674 in the subsequent years. Although this annual increase was partly due to the addition of new sites, for individual districts the mean yearly increase of admissions varied between 61.8% and 68.6%. The most likely explanation for this increase is that increasing confidence of the health care workers and community members in the CLHP resulted in more children being brought for treatment.

  • The average cost per case managed in hospital was US$136 of which the investment cost for the international donor was US$8.34 per case.

  • 10% of children treated were aged <2 mo, 52% were aged 2–11 mo, and 37% were aged 12–59 mo.

  • In children aged 12–59 mo, 2% of cases were nonsevere pneumonia, 62.8% were severe pneumonia, and 25.2% were very severe pneumonia; in infants, 10.0% of cases had severe/very severe pneumonia.

  • The proportion of children dying of pneumonia fell from 18.6% to 8.4%, a reduction of 54.8% over the baseline.
